The more we get to know about you, the better we can serve your dental needs. That’s why, on your first visit, we will allow extra time to learn about you.
We’ll ask you to complete a detailed medical and dental history form as well as some standard consent forms and privacy statements. Next, we’ll need a copy of your dental insurance card, if you have one.
How can we help you?
Once the forms and documents are completed, we’ll step into one of our cozy treatment rooms to talk about ways we can care for you. Tell us about your past dental experiences, your main concerns, your future goals, and any other information which may help us to get acquainted with you.
Next, we will use digital dental x-rays to evaluate your teeth and jaw. Our hygienist will check the health of your gums and recommend the best type of dental cleaning for you.
When you meet Dr. Dean, you’ll be encouraged to ask questions—there’s no such thing as a bad question—and he’ll perform a thorough examination of your teeth and gums. Together, you and Dr. Dean will discuss your treatment needs and goals, as well as plan a strategy for addressing them.

After your time in the treatment room, you’ll visit with our front desk team to schedule your next visit and discuss your payment options.
